IMPORTANT!!!

Beginning with the new session (March 1, 2007)
all players participating in Terminal Hockey Club Skates
will be required to be current active members of USA Hockey,
and have proof of such, in order to get on the ice.

USA Hockey membership is $30.00, and must be paid separately from the Terminal Hockey membership fees.
USA Hockey membership is only on an annual basis, Sept. 1^st. - Aug. 31^st.

Terminal Hockey's contract with the Ice Vault Skating Rink mandates this condition.

Current USA Hockey members MUST provide proof of membership for Club records.

Players WILL NOT be permitted on the ice if Terminal Hockey is not in receipt of a
complete this USA Hockey Waiver PDF Form (click for PDF), and proof of payment in our records.
